I followed all the plots...and yet somehow number 4 confuses me to no end. (partly because i have forgotten)

It says #4 is set 10 years later...so why does snake have a patch like big boss? and he is grey?

Well snake is old because of faulty cloning, and the eyepatch is called the Solid eye system. It goes over his working eye so he can use it like binoculars or NVG and such.
